Every person’s recovery journey is unique, so at River Oaks, we create a tailored plan for your aftercare. Planning for discharge and aftercare programs begins the first week of treatment, so when it comes time to leave the facility, you’ll feel prepared to make the transition into their next phase of recovery.
Discharge and aftercare planning are executed by our team of on-staff case managers who will ensure that each and every patient is prepared to leave the facility and has the tools they need to stay sober and to thrive.
After your treatment has ended, you will receive a completion letter as well as aftercare recommendations, referrals necessary for any aftercare programs, and a client survey. We ask that you rate your experience here at River Oaks, as we are dedicated to continuous improvement of our program so that we serve all our clients in the best manner possible.
Keeping Your Recovery Connection
Staying connected to the recovering community is incredibly important post-treatment. Not only does it help ground you and remind you of your commitment to recovery, but it also helps to minimize your chances of relapse.
As a recovering individual, it is vital to be surrounded by others who know your struggle and are willing to help. Usually, the best option is to have interactions with other recovering individuals, therapists, and doctors, all of whom can help you stay sober in different ways.
Those who agree to check-ins and follow-ups during treatment (and who sign the authorization to conduct follow-ups consent form) will automatically be enrolled in River Oaks’ aftercare program called Outcomes Recovery. Through Outcomes Recovery, we follow up with our patients to see how they’re doing after completing our program and compile relevant information to learn more.
We do not provide your name or information to the public through Outcomes Recovery, and all gathered information is anonymous. We reach out to former River Oaks patients at 2, 6, 12, 18, and 24 months post-treatment. Our hope is to receive honest feedback as well as to check in with individuals living in recovery to ensure that they are thriving.
